Your Medical Records
Your medical record is created on your first visit to Southampton Hospital and will be accessed for any additional visits you might make here, whether for minor surgery, routine diagnostic procedures, laboratory work, physical therapy or any other reason. Your record will also be accessed if you visit any of our convenient satellite care facilities. The medical record number that is first assigned to you will be used for any future medical care you have as an inpatient or outpatient at Southampton Hospital.
The contents of your medical record are confidential and cannot be released without your written authorization. Your personal physician and any specialist you see may ask you to sign a form requesting a copy of your record for them to consult. If you or your guardian should require information from your medical record, you will need to send a written request to our Medical Records Department at:
